CDDO Assessment Coordinator -- Join a mission-driven team making a difference in the lives of Kansans with intellectual and developmental disabilities. We are seeking a dedicated CDDO Assessment Coordinator to coordinate and complete assessments and other reports for annual determination of Intellectual/Developmental Disabilities (I/DD) waiver eligibility for individuals with I/DD in our four county catchment area of Riley, Pottawatomie, Geary, and Clay counties. This role is vital in ensuring accurate assessments of those with I/DD and providing effective communication with individuals, families, and other stakeholders. The ideal candidate will possess strong organizational skills and a background in I/DD services. Duties
  • Performs assessments and verifies individual financial eligibility
  • Maintains accurate, detailed records
  • Schedules and conducts assessments timely and in compliance with HIPAA and state licensure requirements
  • Serves as assessment contact for the CDDO service area
  • Communicates with Targeted Case Management (TCM), Managed Care Organizations (MCO), Home and Community-Based Services (HCBS) providers, and other identified stakeholders
  • Ensure adherence to organizational policies related to assessment and privacy regulations
Skills
  • Excellent communication skills for clear reporting and stakeholder engagement
  • Organizational skills necessary for managing multiple assessments and deadlines
  • Experience working with individuals across diverse age groups and backgrounds
  • Ability to analyze data critically to inform decision-making
Start rate is $21.50 per hour. This is a full-time position and is eligible for our great benefits. What is the CDDO?

Consistent with provisions of the Developmental Disabilities (DD) Reform Act, the Kansas Department for Aging and Disability Services (KDADS) has designated a Community Developmental Disability Organization (CDDO) for every area of the state to help coordinate DD services and supports.

Big Lakes Developmental Center, Inc. (BLDC) as the CDDO for Geary, Riley, Clay, and Pottawatomie counties, works with affiliated agencies and providers to ensure quality services that are cost effective and easy to access.

The CDDO:

-Provides the single point of contact (entry) for people seeking services;
-Maintains an area-wide services needs list for those waiting for services;
-Provides information to assist individuals and their families in determining which service provider(s) they would like to access;
-Works with people towards admission to all needed services;
-Provides information and referral services;
-Coordinates the relocation of people living in state hospitals who wish to return to their home county; and
-Works with affiliated agencies to ensure customer-driven quality service delivery.
